Vitality can mean daily energy, resilience under stress, exercise performance, sexual desire, erectile function, mood, sleep quality, or hormone status. These outcomes overlap in real life, but they are not interchangeable in research.

A trial showing a lower stress score does not prove higher testosterone. A change in testosterone within the normal range does not necessarily improve symptoms. Ashwagandha: Stress Adaptation Is Not the Same as Hormone Treatment

Ashwagandha (*Withania somnifera*) is used in Ayurvedic traditions and is now commonly marketed for stress, sleep, exercise, and male health. Preparations may contain root, leaf, or both and can be standardized to different withanolide profiles. Those differences make the product used in a trial important.

The National Center for Complementary and Integrative Health states that some ashwagandha preparations may help insomnia and stress, while evidence for many other promoted uses remains insufficient. It also describes limited evidence suggesting that use for two to four months may increase testosterone levels and sperm quality. “Limited” is the key word: it does not establish ashwagandha as a treatment for hypogonadism or infertility.

A systematic review of herbal ingredients and testosterone identified positive findings for some ashwagandha and fenugreek extracts, but emphasized small samples, heterogeneous products, mostly younger nonclinical populations, and variable risk of bias. In one randomized crossover study of overweight men ages 40–70 with mild fatigue, a specific ashwagandha extract increased DHEA-S and testosterone relative to placebo, yet fatigue, vigor, and sexual well-being did not show significant between-group differences.

That result demonstrates why a laboratory endpoint and lived outcome must be read separately.

Ashwagandha Safety

Short-term use may be tolerated by many adults, but long-term safety is not well established. NCCIH notes possible drowsiness, stomach upset, diarrhea, vomiting, rare reports of liver injury, and interactions with diabetes, blood-pressure, immune-suppressing, sedative, anti-seizure, and thyroid medicines. It advises avoiding ashwagandha during pregnancy and breastfeeding and cautions people with autoimmune or thyroid disorders, upcoming surgery, or hormone-sensitive prostate cancer.

These are not minor footnotes. A vitality product should not be added casually when the person already has a liver, thyroid, autoimmune, prostate, or medication concern.

Fenugreek: Extract Details Drive the Interpretation

Fenugreek (*Trigonella foenum-graecum*) is a food and botanical ingredient. Male-focused supplements often use concentrated seed extracts standardized for particular constituents. Research on one branded or standardized extract cannot be assumed to apply to fenugreek in every form.

A 2020 meta-analysis of four trials reported an increase in total testosterone with fenugreek extracts. A broader 2023 meta-analysis of seven studies in mostly male athletes found small effects for total testosterone and uncertain effects for free testosterone, body composition, and leg-press performance. The authors also noted heterogeneity.

More recent research continues to show why conclusions remain unsettled. In a 2024 randomized trial, plasma total testosterone and free-testosterone index rose from baseline across fenugreek groups but were not statistically different from placebo for the pooled doses; saliva testosterone showed a different pattern. Different specimens, endpoints, doses, and statistical comparisons can tell different stories within the same study.

The consumer conclusion is not “fenugreek raises testosterone.” It is that selected fenugreek extracts have produced small hormone or training-related signals in some trials, but the clinical meaning, generalizability, and relevance to a particular product remain uncertain.

Fenugreek Safety

Fenugreek can cause gastrointestinal symptoms and allergic reactions, particularly in people with related legume allergies. It may influence blood glucose and may interact with medicines, including glucose-lowering or anticoagulant therapy. Culinary use and concentrated supplement use are different exposures. People who are pregnant, preparing for surgery, or managing medication should seek professional guidance.

Black Ginger: Interesting, but Early

Black ginger (*Kaempferia parviflora*) is a Southeast Asian rhizome rich in polymethoxyflavones. Human studies have explored physical performance and male sexual-wellness outcomes, but the evidence base is much smaller than marketing language often implies.

A randomized trial in 60 soccer players tested 180 mg of a specific black-ginger extract for 12 weeks and assessed several fitness measures. Other studies have used functional drinks or different extracts in broader adult groups. These studies suggest possible performance-related effects, but differences in product, dose, participants, and outcomes prevent a single general conclusion.

For sexual health, a widely cited pilot study enrolled 14 generally healthy men ages 50–68 with self-reported mild erectile difficulty. It was open-label and had no placebo group. Thirteen men completed 30 days and reported improvements on questionnaire scores. That is hypothesis-generating evidence, not proof of efficacy and not a basis for substituting black ginger for established erectile-dysfunction evaluation or treatment.

A supplement containing black ginger should therefore be described in terms of emerging ingredient research, not as a natural erectile-dysfunction drug or a guaranteed testosterone booster.

Maca: Sexual-Wellness Interest Without a Reliable Testosterone Story

Maca (*Lepidium meyenii*) is an Andean plant used as a food and traditional preparation. Products may use different colors, including black, red, or yellow maca, and may be gelatinized, powdered, or extracted. These distinctions complicate comparisons.

A systematic review of randomized trials found limited evidence for improving sexual function, but only four studies met the criteria and the total sample size and methodological quality were insufficient for firm conclusions. Some small trials have reported changes in sexual desire or questionnaire scores without meaningful changes in testosterone.

A 12-week randomized trial in 175 adults at low and high altitude compared black maca, red maca, and placebo. The study reported some changes in mood, energy, and health-status measures, but results varied by altitude, maca type, outcome, and time point. A study like this can inform future research; it cannot support the claim that black maca predictably improves energy or sexual function in every man.

Why the Four Ingredients Cannot Be Added Together on Paper
